Aim and objectives
1. Over the last 5 years has there been any training related to dual diagnosis completed by the mental health nurses based in a&e?
2. To identify nurses level of knowledge and understanding of dual diagnosis in a&e.
3. To action plan with colleagues about ways of enhancing the teams knowledge when in contact with people with dual diagnosis in a&e
4. The findings will be used to inform evidence based recommendations on: professional development for mental health nurses based in a&e for further research.
I work in a&e as a mental health nurse based with the liaison team. The team is multidisciplinary
and includes doctors, nurses and healthcare assessments. Mental health assessments are carried out during day and night time, as the team is on call 24 hours a day. There is increasing number of patients who present to a&e with dual diagnosis, which includes mental health and substance misuse. There is a substance misuse team that are based in a&e during day time hours only. During the night time hours the service is unavailable to provide support or advice to the mental health liaison team. There are always questions raised by both teams about the patients primary need when present with dual diagnosis and which team should take the lead. Patients with dual diagnosis are often frequent attenders to a&e and present out of hours. Their needs may be complex and include social/ health problems. There are occasions when they are admitted to a psychiatric ward due to presenting risk to self or others however some research suggests this is not beneficial and suggests community crisis teams. Mental health nurses are specialists in mental health, and I would question whether they have enough skills and knowledge about substance misuse. Are patients with dual diagnosis Who present to a&e more likely to be admitted to psychiatric ward as mental health nurses have to meet four hour target and may not consider waiting longer if suspect drug/alcohol in system that could affect mood and behaviour.!barriers include; substance misuse not available out of hours for advice. The pressures from a&e to move patients through department within the government requirement of 4 hours.
