~ Furthermore, presenting the ASSESSMENT, DIAGNOSIS and TREATMENT/INTERVENTIONS undertaken for the patient. Critically ANALYSE the DATA gained from patient assessment, monitoring and laboratory findings in relation to change to patient condition and recommended treatment. Tell us how was the patient diagnosed (what was the significance of this intervention/procedure?), any changes in the condition of the patient after particular assessment, how I monitored the patient, what all the alterations as per evidence based practice could have been used or implemented for the patient, if they were not-what was the reason.
